Badge Course Enrollment Request Form

Thank you for your interest in taking a Badge Course and extending your learning. Please complete this form below to request an enrollment.

• Once submitted, you will receive an email welcoming you to your course, generally within one business day. This email will include a link for instant course access.

• You will also receive an electronic invoice via email for the course registration cost. Changes can be made to the invoice once received if needed.

• The electronic invoice will include an invoice payment link, including an option to pay instantly with a card, such as a p-card.

• You will also be able to download the invoice as a PDF. Once payment is complete, you will be able to instantly download the receipt as a PDF as well.

• If you are a tax-exempt organization, check the box below to have no tax applied to the invoice.
